Please read the following important information carefully.

2011 Registration - Update 8/12/11

Registration has closed for all age groups except:

U8-Boys

	LATE REGISTRATIONS will be accepted until May 21st without a late fee. All completed applications along with birth certificates and photographs are to be mailed to the P.O. Box listed below. After May 21st, please also enclose a $30 late fee per child. Incomplete applications and registration packages will NOT be accepted and could potentially delay or prevent the registrant from playing soccer in the 2011 season.
	Birth Certificate Copies Required for ALL NEW PLAYERS and for all players who did not play soccer with Greenhaven Soccer Club last year.
	Registration is open to all youth who are at least 4 years old by July 31, 2011 and younger than 19 years on July 31, 2011. There are NO exceptions to this age rule.
	The Deadline to request a full refund is August 1st The Deadline to request a partial refund (50%) is August 15th Refund checks will be issued September 1st

The following items are needed to register a child to play for GHSC:

1. Completed Greenhaven Soccer Registration Form for each child. Note: Metro players need to use the Select Registration form.

2. A RECENT photo of each child cut to 1 inch square.- NO hats or sunglasses. Please write child’s name and birth date on back of picture. (No picture required for U6.) Click Here for details of acceptable photos.

3. Registration Fees: Cash or checks made to Greenhaven Soccer Club. (There is no discount for more than one child per family.)

U-6 - - $65.00 per child.
U-8 & U-10 - $90.00 per child. (does not include referee fees)
U-12, U14, U16, U19 - $105.00 per child. (Includes referee fee)

NOTE: A referee fee is included in the cost of registration for all children playing in age groups U-12 and above (U-12, U14, U-16 and U-19), and in Select. A referee fee is not included for recreation players playing in U-8 and U-10 age groups and will need to be collected by each team. (No referee fee for U-6)

4. A completed California Youth Soccer Association (CYSA) membership form (U8-U19)--2 copies OR US Club Soccer registration/membership form for U6)--2 copies. Please note: Each copy must be signed.

5. A completed 2011 Volunteer Service Form. Each family is expected to volunteer time to the club. Please see the new 2011 Volunteer Policy.

6. A government issued birth certificate for all players that are either new to Greenhaven Soccer Club or did not play in 2010.
